Immigration Physicals

The following information is for individuals applying for their green cards who need the I-693 form completed.

Please Note: Due to the large number of people who call to schedule green card exams, there will be a $100 fee if you do not show up for your appointment.

Updated Fees

Ages 18 and older:

$300.00; includes exam, paperwork, and lab tests

Ages 17 and younger:

$150.00; includes exam, paperwork, and lab tests

Important Notes:

  1. Fees do not include vaccines or a chest Xray if the TB test is positive
  2. An interpreter can be arranged if the appointment is scheduled at least one week in advance
  3. Test results can take an average of 7-10 days

Required Tests:

Ages 2-17 years old: TB

Ages 18-24 years old:  TB, RPR (syphilis), gonorrhea

Ages 18-44 years old:  TB, RPR

Ages 45 and older:  TB

Required Vaccinations

Birth-18 years old:  All age-appropriate vaccines must be up to date, including for COVID-19(1)

19-64 years old:  Tdap, MMR(2), Varicella(2), Hepatitis B(2,3), COVID-19,Polio, Flu(4)

65 and older:  Tdap, MMR(2), Varicella(2), COVID-19, Polio, Flu(4), Pneumonia(5)

  1. COVID-19 vaccine only required for ages 6 months and older
  2. MMR, Varicella, and Hepatitis B blood titers can be checked if no vaccine records are available
  3. Hepatitis B not required for applicants age 60 and older
  4. Flu vaccines are only required between October 1- March 31
  5. Pneumonia vaccine requirements as follows: PCV15, but if it was given over a year ago, must also get PPSV23

Please bring the following to your appointment:

A photo ID such as a passport, state ID, or driver’s license
Any vaccine records
Previous TB (tuberculosis) test results
Previous lab test and X ray reports

Vaccines for kids must be up to date
Vaccines required for adults- Tdap, MMR, Varicella (if you did not have chicken pox)
Flu vaccines are required between October 1- March 31
All vaccines can be done at one of the pharmacies, urgent care centers, or the health department (with an appointment)

If no payment is received within 30 days of completing the required labs, the patients (applicants) will be responsible for paying LabCorp for these tests.
