Immigration Physicals

The following information is for individuals applying for their green cards who need the I-693 form completed.

Updated Fees

Ages 18 and older:

$250.00- includes the exam, paperwork, fees for the 3 required tests (syphilis, gonorrhea, and TB).  

If any additional lab tests are needed (such as titers for varicella and measles, mumps, and rubella), the fee will be $300.00.

Ages 17 and younger:

$125.00- includes the exam, paperwork, and fee for the 1 required test (for TB)

Important Notes:

  1. Fees do not include vaccines or a chest Xray if the TB test is positive
  2. An interpreter can be arranged if the appointment is scheduled at least one week in advance
  3. Test results can take an average of 7-10 days

Required Vaccinations

  • Ages 19 and younger must be up to date on all of their vaccinations
  • Ages 20-64 need a Tdap vaccine
  • Ages 65 and older need Tdap and pneumonia vaccines
  • *Flu vaccines are required between October 1- March 31
  • *Covid vaccines are required for ages 12 and older
  • You can apply directly with USCIS for a vaccine waiver. However, it must be for all vaccines, not only one.

Citizenship Waiver Exams - $500.00; includes 2 office visits and completion of Form N648

Please bring the following to your appointment:

A photo ID such as a passport, state ID, or driver’s license
Any vaccine records
Previous TB (tuberculosis) test results
Previous lab test and X ray reports

Vaccines for kids must be up to date
Vaccines required for adults- Tdap, MMR, Varicella (if you did not have chicken pox)
Flu vaccines are required between October 1- March 31
All vaccines can be done at one of the pharmacies, urgent care centers, or the health department (with an appointment)

If no payment is received within 30 days of completing the required labs, the patients (applicants) will be responsible for paying LabCorp for these tests.
